Professional and Institutional 5010 Claims for the payers listed below may have received the following invalid error messages on the Payer Claim Data Report (SR) due to a payer processing issue:

A3:453 ACK,RETURND AS UNPRCESSBLE CLM- CLM,ENCNTER BEEN REJECTD AND NOT ENTERD INTO ADJUDICATION SYS.: Procedure Code Modifier(s) for Service(s) Rendered

A3:454 ACK,RETURND AS UNPRCESSBLE CLM- CLM,ENCNTER BEEN REJECTD AND NOT ENTERD INTO ADJUDICATION SYS.:PROCEDURE CODE FOR SERVICES RENDERED.

This issue began on July 11, 2012. Cigna is currently working to correct this issue and once the issue is resolved any affected claims will be resubmitted. Cigna apologizes for any inconvenience.

CPID 4509 CIGNA Health Plans
CPID 6405 CIGNA Health Plans
