The following payer has changed their Remittance agreement form due to a routing change:
•3815 Physicians Health Plan of Northern Indiana - Professional
•3596 Physicians Health Plan of Northern Indiana - Institutional
Effective immediately, providers already approved to receive remittance transactions through the clearinghouse must complete a new agreement form.
To access the new agreement form, please visit our website at http://www.collaborationcompass.com, click Payer, and Payer Agreements Library.
Action Required: Already approved providers must re-enroll for this payer to continue receiving remittance. All providers should begin using the new agreement form in the Payer Agreements Library immediately.
