Due to a payer intermediary processing issue, Professional and Institutional claims for the payers listed below may have received the following invalid error message on the Payer Claim Data Report (SR), Payor Claim Data Report (SE), Payor Report Data File (SF):

•A3:54 Duplicate Claim

This issue began on 10/04/2013 and was resolved by the payer intermediary on 10/08/2013.

Payer intermediary apologizes for any inconvenience.

Payers affected:

Providers should ignore reports with duplicate submission errors if they have received accepted payer reports
