Professional Claims for CPID 2414 Horizon Blue Cross Blue Shield of New Jersey may have rejected inappropriately due to a payer processing issue with the error message:

CLAIM REJECTED LINE LEVEL REND PROV REQ WHEN BILLING/PAY TO PROV=GROUP AND CLAIM LEVEL REND NOT PRESENT. ACKNOWLEDGEMENT/RETURNED AS UNPROCESSABLE CLAIM-THE CLAIM/ENCOUNTER HAS BEEN REJECTED AND HAS NOT BEEN ENTERED INTO THE ADJUDICATION SYSTEM.

This issue began on December 12, 2011 and has been corrected as of January 4, 2011. All affected claims will be resubmitted to the payer.
